    Smallpox killed and disfigured across eighteenth-century Britain. Lady Mary Wortley Montagu had earlier publicised Ottoman inoculation (variolation) in the 1720s: deliberate infection with mild smallpox, which was risky.

    Edward Jenner, a Gloucestershire doctor, tested the folk claim that cowpox protected milkmaids. In 1796 he inoculated James Phipps with cowpox, then challenged him with smallpox material; Phipps did not develop smallpox.

Attitudes can block a proven method. Gillray is evidence of fear and ridicule, which is why later Acts had to compel vaccination rather than wait for consent.

Once smallpox is framed as a neighbour’s threat, laissez-faire looks selfish. The Act is government as a factor, not Jenner working alone in Berkeley.

Chance, observation and the missing theory

Jenner is often taught as a eureka farmyard tale. Tighten it. Variolation already existed; dairy folklore already existed; Jenner’s contribution was a safer, testable protocol and publication. He still explained cowpox in humoural and local-inflammation language. That is why this is not “the discovery of germs”. The consequence was huge for one disease: smallpox deaths fell where vaccination was widespread. The limitation was conceptual: without Pasteur’s later work (1881 anthrax and rabies vaccines), doctors could not systematically culture other protections. In essays, place Jenner as a turning point for prevention of smallpox and as a reminder that chance plus observation can outrun theory — then show government spreading the needle.

From voluntary needle to the 1853 Act

A method on a Gloucestershire farm does not vaccinate a nation. Arm-to-arm transfer, lymph supply, and parish organisation all mattered. The 1853 Vaccination Act, passed in the shadow of epidemic fear and a more statistical state, made the infant needle a legal duty. Resistance was not only “ignorance”: some objected to state intrusion into the home, some to unclean technique that actually spread infection, some to class — working people fined for non-compliance. The 1898 conscience clause shows attitudes forcing the law to bend. Compare this with the permissive 1848 Public Health Act: prevention of smallpox went compulsory earlier than urban sewers. That comparison is excellent 16-mark material on government priorities.

Question: “Jenner’s vaccination was the biggest breakthrough in nineteenth-century medicine.” How far? Judgement: it was the biggest breakthrough in preventing one killer, not in explaining disease. Support: 1796 experiment, 1798 publication, 1853 compulsion, later eradication. Challenge: Pasteur’s germ theory from 1861, Koch’s specific microbes, Lister’s 1867 antiseptics and the 1875 Public Health Act changed more diseases and surgery. Conclusion: Jenner is a giant for smallpox; germ theory is the bigger hinge for medicine as a whole because it told doctors what they were fighting.
